Disability File
What does Disability File mean?
The disability file is an electronic file created by the SSA with information about the disability claimant. Although the file used to be an actual file, currently the information is stored electronically. Claimants who need their disability file can request a copy of their CD. Copies of disability files can be requested by completing a Release of Information (Form 3288) and sending it to the Social Security Administration, although the request may not include certain mental health records. One C.D. should be provided free of charge, but there will be a cost for multiple copy request.
If you have hired a disability lawyer they can also request the disability file by completing form SSA 1686 (which designates a claimant's representative). Consider also, you have the right to review your file which can be very helpful, especially if you have been denied benefits and you are filing an appeal for an administrative hearing.
